Getting Started

Starting services should feel clear, respectful, and organized. SLCRC begins by learning about the person, their goals, their daily support needs, and the team around them.

How service conversations begin

  1. Share the person's goals, service interest, location, schedule, and support needs.
  2. Talk through daily living needs, health and safety considerations, and team expectations.
  3. Identify whether RHS, Supported Living, or another approved support area may be appropriate.
  4. Discuss staffing, timing, and practical next steps.
  5. Confirm any commitments only after leadership review and team coordination.

Helpful details to share

  • What kind of help is needed during a typical day or week.
  • Health, safety, behavioral, medical, supervision, transportation, and ADL needs.
  • Guardian, family, care-team, and decision-maker contacts.
  • Preferred schedule, location, communication needs, and personal goals.

The right support starts with a real conversation. SLCRC takes time to understand each person's goals, support needs, living preferences, and team expectations before confirming the best path forward.
